A Thoughtful, Individualized Approach to Home and Senior Living Decisions
Deciding whether to remain at home or make a move can involve important questions about health and safety, identity and independence, family circumstances, finances, and personal preferences. Salem Elder Care brings expertise and objectivity to the decision, helping older adults and their families understand their options and determine the best path forward.
Holistic Assessment
We begin by understanding what has prompted the family to consider a change and the questions and concerns surrounding the decision. A comprehensive assessment then considers the older adult’s physical, cognitive, emotional, and social needs, along with their current living environment and available support, providing the foundation for thoughtful, individualized recommendations.
Recommendation and Decision Support
Following the assessment, we provide clear, objective recommendations about the living situation and level of support that will best meet the older adult’s needs. If remaining at home is appropriate, we identify the care, services, resources, and home modifications needed to support safety and independence and can help put them in place.
If a move is recommended, we help determine the level of care and type of living environment best suited to the older adult’s individual needs. Drawing on our knowledge of local senior communities, we identify those that best match the older adult’s needs, preferences and budget, and provide an experienced, objective perspective as they make their decision.
Move Coordination and Follow-Up
When a move is the right choice, we help prepare the older adult for the transition, providing support through the practical and emotional aspects of the change. We connect families with trusted moving and downsizing professionals, coordinate needed services and logistics, and remain involved after the move to support the older adult’s adjustment and ensure the new living environment is meeting their needs.
Questions About Move Mangement Services?
There is no single answer, and that uncertainty is one reason many families benefit from a professional assessment. Common signs that it may be time to consider a change include safety concerns at home, increasing isolation, difficulty managing daily activities, or care needs that are becoming harder to meet in the current setting.
A comprehensive assessment looks at the whole picture and provides clear, objective guidance—whether that means recommending a move or identifying the additional support, services, and modifications that could make remaining at home a good option.
