Home or Senior Living Assessment

Older adults and the people who care about them may face difficult questions about whether remaining at home or making a move is the best fit for their needs and circumstances. I help assess which setting—home or a senior living community—can best support safety, day-to-day needs, and well-being. If a move becomes the right next step, I can help with the decisions and preparation involved, so the older adult’s preferences and priorities remain at the center of the process.

Caregiver Assessment

When one person is carrying much of the responsibility for someone else’s care, it can be hard to see what is working, what is becoming too much, and where help may be available. I take a thoughtful look at the caregiving situation—including the caregiver’s responsibilities, well-being, available support, and the older adult’s needs—and offer practical recommendations for what may help next.

Quality of Life Assessment

As health or abilities change, it can become harder to stay connected to the activities, relationships, and experiences that bring enjoyment to daily life. I help older adults identify what has become more difficult to access and explore realistic, creative ways to remain involved in the things that are important to them.

Hospital Advocacy & Support

When an older adult is hospitalized—whether the stay is planned or unexpected—the medical system can feel overwhelming and difficult to navigate. I offer advocacy and a reliable point of contact, helping the older adult and the people close to them communicate with the care team and prepare for discharge. I can also remain involved afterward, helping keep attention on the care, follow-up, and day-to-day support needed after the hospital stay.
